The CCI/GHC Project Program is
based on meeting quality and healthcare
management measures.

The Physician Component includes meeting the
following Measures:

  • Care Opportunities with well-managed diabetes
  • LDL control
  • LDL control
  • Glaucoma Screening
  • Generic Rx Prescribing
  • Risk Adjusted Payments (RAPS) collected Data

The Hospital Component includes:

  • Improve Care for MI
  • Prevent Surgical Site Infection
  • Percentage of Members Receiving Pneumonia Screening
  • Patient Safety
  • Risk Adjusted Payments (RAPS) Collected Date
