About Us
The Greater Hartford Coalition is committed to establishing
regional, evidence-based approaches to providing, measuring
and improving patient care, education and health outcomes
for the greater Hartford community.
Utilizing electronic data-sharing tools among
providers and patients, the Coalition will focus on:
- measuring performance (process and outcomes)
- identifying opportunities for improvement
- encouraging collaborative care management among providers and between providers and patients, including patient education
- implementing changes that lead to more consistent, efficient and cost-effective care delivery.
